Case Management . . . for Community Living
Maximum self-sufficiency is the goal for every client. Through the Case Management
program, Skyland Trail offers independent living support for clients who are ready
to take the next step for community integration. Guided by staff assigned to the
client, the individualized support is tailored to their needs, enabling growth in
self-management. In a general sense, the Case Management program emphasizes
prevention, a healthy
lifestyle,
and community involvement.
Admission to the program depends on a client's ability to maintain independent
living. Some clients enter after graduating from the 24-hour residential treatment
program. Others enter the program directly, while living at home with family or in
their own residence. As part of the program, clients are expected to have a
structured daily schedule, which may include Skyland Trail's day treatment program,
school, employment, or volunteer work.
The core services of the program include 24-hour on-call staff, weekly visits with
staff, medication monitoring and management, mental health status checks, treatment
planning, monthly community meetings, assistance with accessing community resources,
supportive counseling for specific needs, and family meetings.
