Transitional Housing
Transitional Housing is an apartment-based program featuring an array of
services to foster illness management and reintegration into family and
community. It provides clients the opportunity for independent living,
supplemented with other therapeutic services (most often Case Management or Day
Treatment and LEAP). This level of care is
typically utilized by clients for six months or longer and provides for a
social support network in a congregate setting.
Transitional Housing is offered to clients who require a therapeutic living
arrangement that is less intensive than 24-hour residential services. The
apartments are safe, well maintained, affordable, and accessible. Clients
in this program are responsible for their own transportation and
nutritional needs and for filling and administering their own medications.
Supplemented with a minimum of our Case Management program, the client can
expect in-home visits and a twenty-four hour, seven day a week emergency
call staff resource.
